Job Description

Established in 1980, the Greater Lawrence Family Health Center (GLFHC) is a multi-site mission-driven non-profit organization employing over 700 staff whose primary focus is providing the highest quality patient care to residents throughout the Merrimack Valley. Nationally recognized as a leader in community medicine (family practice, pediatrics, internal medicine, and geriatrics), GLFHC has clinical sites throughout the service area and is the sponsoring organization for the Lawrence Family Medicine Residency program.

GLFHC is currently seeking a Community Health Worker, Medical Assistant for our Office Based Addiction Treatment (OBAT) program. The OBAT CHW/MA is part of an integrated team that addresses the clinical and social needs of OBAT patients. This position supports the OBAT RN by performing nursing support and clerical tasks, case management, outreach, education, social support and linkages to appropriate supportive services through coordination and navigation.

  • Provides support to nursing during the screening, intake, induction, stabilization, and maintenance phases of office based opioid treatment.
  • Meets potential new patients with Substance Use Disorder in the Emergency Department or Lawrence General Hospital under the direction of the RN or MD for initial screening, including insurance verification, intake process and assistance with coordinating care post-acute care.
  • Educates patients on overdose prevention and infectious disease prevention.
  • Remains current on all referral sources, including but not limited to: housing, nutrition services, peer programs, Mass Health/Healthy Start, advocacy services, transportation, parenting skills and groups, prenatal classes, substance abuse and mental health services including methadone maintenance, domestic violence, employment and training, nursing case management, and primary care.

Qualifications

  • Graduate of a Medical Assistant program (or evidence of education and experience working in a foreign country as LPN or RN).
  • Degree in Human Services preferred or combination of education and experience in community based service delivery considered.
  • Bilingual, bicultural (Spanish/English) required.
  • Community Health Worker Certificate or must complete with 12 -18 months of hire.
  • Become a Certified Application Counselor within 2 months of hire.
  • Computer proficiency, especially in Microsoft Word, Excel and Outlook.
  • Valid Massachusetts Driver’s license and access to reliable transportation.
  • Current CPR/BLS Certification.
  • Ability to work evening hours.
