Job Overview:
The Clinical Pharmacist is a key member of the interdisciplinary health team, responsible for optimizing medication use for patients with mental health, substance use disorders, and chronic pain management and other chronic health conditions. This role includes the coordination and management of long-acting injectable (LAI) antipsychotic and psychiatric medication programs, providing direct patient care through Medication Therapy Management (MTM), and conducting Comprehensive Medication Reviews (CMRs) to improve outcomes, safety, and adherence.
Duties:
Medication Therapy Management (MTM) & Comprehensive Medication Reviews (CMR):
- Conduct MTM and CMRs for clinic patients to identify and resolve drug therapy problems.
- Document and communicate recommendations to the healthcare team and follow up as needed.
- Facilitate prescription management to appropriate pharmacy based on insurance payer and patient specific needs.
- Support tapering efforts and education regarding polypharmacy management in complex cases.
- Manage billing processes for services provided, ensuring compliance with payer requirements and securing appropriate reimbursement
Patient Education and Counseling:
- Provide individualized patient education focused on medication use, side effects, adherence strategies, and safety.
- Address stigma, misconceptions, and concerns related to psychiatric, pain, and substance use disorder medications.
- Provide counseling, as required, for prescriptions dispensed through the lockers, ensuring patients understand proper medication use, side effects, and storage instructions.
Collaborative Care & Clinical Integration:
- Participate in behavioral health huddles, case conferences, and treatment planning sessions.
- Act as a clinical resource for psychiatric medication management to providers and staff.
- Engages with pharmacy staff as part of an interdisciplinary team to support comprehensive, patient-centered care
- Coordinate and manage the scheduling, administration, and follow-up of LAI antipsychotics and other psychiatric injectables.
- Work collaboratively with behavioral health providers, nursing staff, pharmacies, and care coordinators to ensure continuity of care.
- Monitor and document patient response, adherence, and any adverse effects of injectable therapies to support specialty accreditation requirements.
Quality, Safety, and Compliance:
- Ensure compliance with state and federal laws and standards of care.
- Participate in quality improvement initiatives related to medication use and behavioral health services.
- Maintain accurate records of services provided for billing and audit purposes.
- Identify quality care gaps for patients and work collaboratively with clinical care team to address
- Identify, monitor, and report key metrics that address the impact of this role on overall patient care and satisfaction.

Requirements
- Doctor of Pharmacy (PharmD) degree from an ACPE-accredited program.
- Current pharmacist licensure in Missouri or ability to obtain prior to hire.
- Strong knowledge of psychiatric pharmacotherapy and psychotropic medications.
- Excellent communication, documentation, and interpersonal skills.
Preferred:
- Completion of a PGY1 residency in Psychiatric Pharmacy or Board Certification in Psychiatric Pharmacy (BCPP).
- Experience working in behavioral health or integrated care setting.
- Familiarity with LAI program development and management.
- Experience providing MTM/CMR services in an outpatient or ambulatory care setting.
