Position Overview:

We are seeking a compassionate and experienced Clinical Supervisor to join our Hospice team. This hybrid role combines both field-based clinical responsibilities and office-based leadership responsibilities to ensure the highest quality of care for our patients and families. The Clinical Supervisor (Hospice) will support and guide the interdisciplinary hospice team, assist with case management, and ensure compliance with regulatory standards and internal policies.

Responsibilities:

  • Provide clinical supervision and support to hospice field staff, including RNs, LVNs, CHHAs, and other interdisciplinary team members.
  • Participate in patient care visits as needed, complex cases, or staff training.
  • Conduct case conferences and ensure timely coordination of care plans.
  • Collaborate with the Director of Hospice and DON to ensure compliance with hospice regulations, policies, and procedures.
  • Monitor quality of care and documentation for accuracy and completeness.
  • Assist with staff education, orientation, performance evaluations, and competency assessments.
  • Act as a clinical resource for field staff, patients, and families.
  • Participate in after-hours on-call rotation as needed.
  • Support quality improvement initiatives and prepare for accreditation surveys or audits.

 

Qualifications:

  • Current Registered Nurse (RN) license in California required.
  • Minimum of 2–3 years of hospice or palliative care experience.
  • Prior leadership, supervisory, or case management experience preferred.
  • Strong knowledge of Medicare hospice regulations and end-of-life care standards.
  • Excellent clinical judgment, organizational, and communication skills.
  • Comfortable with electronic medical records (EMR) and proficiency in basic office software.
  • Valid driver’s license, auto insurance, and reliable transportation required for field visits
