Job Description
Summary:
The Community Education Liaison is responsible for coordinating any/all communication, treatment plan information, assessments, and related information between the Child/Adolescent Partial or Intensive Hospital Outpatient Programs and the schools of all patients attending day treatment. This role assists the patient's treatment and transition to post treatment.
Job Duties & Responsibilities:
Serve as a resource, both internally and externally, regarding educational placement issues of the clients of Partial or Intensive Outpatient Programs with local schools.
Direct involvement in the development of patient programming.
Complete the initial admission duties, attend school staffings, IEP meetings and conferences for designated patients in either PHP, IOP or inpatient level of care and document in the medical record.
Alert and keep updated school personnel, parents/guardian, patient aligning student and patient expectations of attendance and completion of work. Document progress and complete and Education Assessment Plan.
Develop an educational plan with the school regarding behavior management of the patient at school.
May be asked to perform in-classroom observations.
Local travel required to schools.
Act as a direct resource for school personnel to contact regarding patient's treatment; document all contacts in the medical record.
Provide crisis intervention consultation via phone regarding assigned patients.
Act as a direct resource for parents and guardians regarding school issues of patients.
Understand special education laws and how they can assist assigned patients and their parents in the school setting
Assist parents in navigating the IEP or 504 process.
Coordinate discharge planning regarding the educational placement and needs of the patients.
Complete school safety plan with assigned patients and discuss with schools to assist in patient's reintegration into school setting
Arrange and conduct discharge staffing with the schools.
Attend staffings, communicate with the multidisciplinary team, and provide input for the treatment plan.
Add education goals and interventions to assigned patient's treatment plan
Document in plan as required by regulatory standards
Attend weekly supervision with clinical supervisor
Collaborate with the Outreach department and assist with the marketing of the Child and Adolescent programming
Provide contact information to the Marketing department.
Provide information regarding Rogers Behavioral Health to schools.
Lead tours of the facility for visiting school staff.
Gather information of needs of the school community and communicate to Community Liaisons.
Performs other duties as assigned within the education, license, experience and training.
Additional Job Description:
Education/Training Requirements:
- Bachelor's Degree in human services, psychology, social work, or related field required.
- Four years of experience working with children/adolescents with mental or behavioral health concerns (special education; autism; etc…)
- Working knowledge of IEP and 504 processes.
- Formal training in the management of the aggressive patient (NCI) is required within sixty (60) days of date of hire. Annual re-certification is required.
- American Red Cross Professional Rescue is required within thirty (30) days of date of hire. Bi-annual re-certification is required.
- Attend all mandatory in-services and other in-services relevant to the position. Be responsible for all educational requirements and related obligations pertaining to the license of the program/unit license regulatory requirements governing the program(s).
- Hold a valid driver's license in the state in which they are employed. Required to have driving record approved by the Hospital's insurance carrier.
