Job Description

Community Health Worker

Are you passionate about empowering people to live healthier lives? Join us as a Community Health Worker and help bridge the gap between healthcare and the community. Under the guidance of the Community Health Manager, you'll deliver health and wellness education to employees, underserved populations, and the broader community - making a real difference in helping individuals adopt healthy behaviors.

What You'll Do

  • Facilitate health promotion and wellness education programs for employees and community members.
  • Support underserved populations by providing resources and guidance to encourage healthy lifestyle changes.
  • Collaborate with the Community Health Manager to implement initiatives based on the Community Health Needs Report and Employee Health Risk Assessment.
  • Travel independently within the community to connect with diverse groups and deliver impactful outreach.

What We're Looking For

  • Education: Certified Nursing Assistant or Medical Assistant from an approved program (or equivalent). A.A. degree or higher preferred.
  • Experience: At least 1 year in health, education, or community outreach preferred.
  • Licensure/Certification: Eligible for Community Health Worker certification within the first year.
  • Driver's License: Valid license required; clean driving record preferred.

  • Strong interpersonal, presentation, and facilitation skills.
