Offered Rates:
- PT/OT - $50/hr
- COTA/PTA - $31-32/hr
Pre-employment documents that you need to email/fax:
- Up-to-date Resume
- Valid IDs (Driver License, Passport, Resident Card, Employment Authorization Card)
- Social Security Card
- PT/OT License/Registration
- Two reference letters
- Physical Health exam results with PPD, MMR lab results
- Course Completion Certificate/Diploma
- BLS/CPR Card (optional)
- Voided Check/Direct deposit form for payroll
