Job Description
Haven Homes, a Cassia community, is currently seeking a Registered Nurse (RNs) to join our team as an MDS Coordinator. As an MDS Coordinator at Haven Homes, you will manage the clinical care of residents by assigning different tasks to nurses within the department. Our ideal candidate should have previous MDS experience, thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills.
Position Type: Full-Time
Pay: $35 - $45 / hour depending on experience.
Bonus: $10,000

MDS Responsibilities:
- Complete MDS and care planning.
- Complete all documentation for RAI and MDS assessments and observations.
- Help develop initial and temporary care plans.
- Work in collaboration with Admissions staff, Case Manager and other clinical staff.
- Perform quality assurance audits to ensure compliance with state & federal regulation in all departments.
- Assist in preparation for state and federal surveys.
- Document and maintain paperwork, referral records, and reports.
- Part of nurse call rotation.
- Other duties as assigned.
MDS Qualifications:
- Current MN Registered Nurse (RN) license required
- TCU, LTC, and MDS experience preferred.
- Proficient in MS Word, Excel, and MS Outlook
- Strong communication and customer service skills
- Strong clinical background and knowledge of LTC rules and regulations.
