Job Description

I.    JOB SUMMARYThe RN Diabetes Educator will provide group and/or individual diabetes self-management education/training to patients and family members, so as to enable them to learn techniques to bring their disease under control, avoid potential complications and bring about lasting improvements in quality of life.  Educational topics may include blood glucose monitoring, behavior modification, nutritional/exercise concepts, dietary modification/meal planning, insulin self-management, oral medicine self-management, diabetes care during illness/pregnancy, and insulin management.

II.    ESSENTIAL JOB RESPONSIBILITIES

  • a.    
  • Assesses and evaluates individuals with diagnosed diabetes (Type 1, Type 2, gestational), to identify knowledge deficits and assess education needs.
  • b.    
  • Reviews physician’s referral information related to patient’s diagnosis and related medical conditions/medications.
  • c.    
  • Identifies the patient’s specific diabetes self-management and medical nutrition therapy goals, and develops unique educational care plan to best meet individual needs and lifestyle.
  • d.    
  • Confers with provider regarding patient’s individual care plan and progress.
  • e.    
  • Establishes and records baseline data for use in monitoring and evaluating going progress.
  • f.    
  • Involve patients and staff in the development and planning of health education materials and activities.
  • g.    
  • Maintains and applies current knowledge of diabetes disease processes and continuous advances in diabetes understanding/treatment and self-management principles, standards, practices and tools.
  • h.    
  • Provides patient instruction for use of glucose monitor.
  • i.    
  • Maintains excellent customer service.

    III.    JOB QUALIFICATIONS

    Required:  o    Current Kansas Registered nurse Licenseo    Two years clinical experienceo    Current BLS certificationPreferred:o    Diabetic Education experienceo    Certified Diabetes Educator (CDE)
