Samaritan Healthcare is seeking a Director of Care Transitions for our growing community! The Director of Care Transitions is accountable for the daily operational management of departmental functions related to Utilization Management and Transitions of Care. This leadership role provides direct oversight of RN Care Managers, Agency-Affiliated Counselors/Social Workers, Case Management Assistants, and Utilization Review RNs. The position is integral to ensuring the efficient utilization of clinical resources and supporting activities aligned with Conditions of Participation. Additionally, the Director of Care Transitions offers guidance and direction to House Supervisors regarding patient status determinations.
This is a full-time salary exempt position.
DIRECT & INDIRECT REPORTS
- RN Care Managers, Agency Affiliated Counselor/Social Worker, Case Management Assistant, and Utilization Review RN's
EDUCATION & EXPERIENCE
Education:
Graduation from an accredited school of nursing preferred or a Master's prepared social worker.
Bachelor's Degree required.
If RN, current Washington State RN Licensure required within timeline required by the WA department of health.
Certification:
CCM or ACM Certification in Case Management strongly preferred.
Experience:
Three (3) to five (5) years acute care clinical experience.
If a registered nurse, experience in a variety of acute care nursing departments, preferred.
One (1) year Utilization Management/Case Management experience, preferred.

Responsibilities include maintaining flexibility in assignments, ensuring compliance with applicable regulations, and exercising sound judgment to support safe, effective, and accountable operations.
PHYSICAL REQUIREMENTS:
- Frequent prolonged standing/walking. Occasional lifting, reaching, kneeling, bending, stooping, pushing and pulling.
- Heavy physical effort, lift/carry up to 50+ lbs. Strength is required to effectively use physical restraints.
- Good reading eyesight; full visual acuity, depth perception, and color perception.
- Ability to communicate using verbal and/or written skills for accurate exchange of information with physicians, nurses, health care professionals, patients and/or family, and the public.
