Job Description
Offering a $5,000.00 sign on bonus payable at 30 and 90 days.
Responsibilities:
- Conducts initial assessment of prospective residents, including off-site visits
- Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident
- Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents
- Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
- Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members
- Hires, trains, schedules, and leads the clinical team in providing exceptional care experiences
- Coordinates the Community's infection control program while maintaining state regulatory compliance
- Develop and oversee the Community's medication management program
- Contributes toward quality assurance and performance improvement initiatives
- Complies with all rules and regulations regarding confidentiality and privacy of resident information
