Job Description

- Assess & Plan
- Complete in-home assessments and reassessments; craft and update individualized Care Plans with client/family approval.
- Meeting all deadlines for re-assessments and/or competency training.
- Coach & Supervise
- Provide training to caregivers to ensure quality and consistent care.
- Positive and approachable but unwavering on clinical expectations for aides.
- Pro-active notes/documentation correlating with drop-ins and level up training.
- Monitor & Document
- Track vitals, condition changes, and interventions in our HIPAA-secure EMR; escalate concerns promptly.
- EMR oversite with strong proactive documentation skillset.
- Strong ability to set schedule to meet deadlines while maintaining flexibility in last minute changes for new clients or quick SOC.
- Collaborate & Advocate
- Coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
- Align strongly with Just-In Home Care Business Development Director collaborating on opportunities to manage care aligned with our often-skilled service provider synergistic partners.
- Understand and articulate mission while aligning with company cultural values in a strong exemplary way.
- Quality & Compliance
- Maintain audit-ready client files, collect initial service payments as needed, and uphold state home-care regulations.
- Active Registered Nurse (RN) license in Maryland
- Bachelors in Nursing or related human-services field OR2+ years’ geriatric/home-care experience
- Leadership abilities in two main areas; caregiver training/clinical growth and referral source collaboration.
- Solid clinical assessment skills and a passion for problem solving
- Lead on all RSA regulation, interpretation, and steer the company in all thing’s compliancy.
- Warm, professional communication that builds trust with families and field staff
- CPR/First Aid certification and a valid driver’s license
- Harford County Resident (not required) but strongly preferred
- $77,500 – $87,500 salary range (experience considerations)
- Paid Holidays (worked into salary)
- Mileage reimbursement2 weeks paid vacation
- Incentive pay available based of KPIs. (available after 6 months of employment performance considerations)
- Active hours/expectations: 8A – 5P Monday through Friday, and flexible evenings and weekends (Just-In Home Care) will not be the only employed nurse so not every assessment or need will be expected to be filled specifically on evenings and weekends.
