Job Description

EDUCATION, LICENSE, CERTIFICATION and EXPERIENCE:

  1. Education High School Diploma or equivalent preferred.

  2. Licenses No professional license required.

  3. Certifications No professional certification required.

  4. Experience 1-2 years related experience required or preferred.

REQUIRED SCREENINGS:

  1. Drug Screen

  2. Physical Assessment

  3. Tuberculosis screen

  4. Background check (Criminal, Civil, Educational, Previous Employment, etc.)

  5. Driver Record screen (positions requiring on-the-job driving)

ESSENTIAL FUNCTIONS:

  • Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
  • Communicate necessary information to others as appropriate.
  • Conduct screenings to evaluate the needs of our patients
  • Assist with referring patients who may require additional care or resources including primary care
  • Must be able to adapt and maintain in an environment with multiple distractions
  • Attends and participates in Quality meetings and implementing measures
  • Communicates effectively with providers, families and other staff
  • Compliance with governmental regulations, and complies with OSHA, HIPPA and CILA regulations
  • Facilitates IDT meetings on Fridays and other job duties as assigned

GENERAL DUTIES:

  • Maintains patient/employee privacy and confidentiality at all times.
  • ON Call rotation
  • Proficient knowledge of the EMR and other platforms used to facilitate discharge planning

Wage starts at $21.63 and goes up with experience

Monday- Friday 8 am- 4:30 pm

Immunizations required for employment, including FLU vaccine
