LUTHER HALL Nexus-PATH is looking for a full-time Nurse (LPN)!

The LPN is responsible for providing education to clients, parents, and guardians on medications, conditions, and other topics as deemed appropriate. This position is also responsible for ensuring accurate documentation in compliance with State, Federal, and Accreditation standards. Follows the North Dakota Nurse Practice Act, and under the supervision and delegation of Registered Nurse or other advanced practice medical professionals.

The Licensed Practical Nurse (LPN) is responsible for tending to the medical needs and concerns of clients to support the delivery of effective and efficient care in a 24-7 residential care in a Psychiatric Residential Treatment Facility working to create a healthy environment to bring about positive changes in behaviors, attitudes, and values of youth residing in the facility. This position conducts assessments, administer medication, and effectively communicate with the treatment team to maximize medical care for clients.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Medical Service- Administer medications and medical care to clients; Fill medication orders with pharmacies; Schedule and coordinate medical appointments with providers and schools for clients; Assess health needs of clients based on needs, concerns, and/or requests including vitals, physical, dental, immunizations, lab work, and visual; Assess client behavior for medication effectiveness and/or side effects; Ensure medical errors are addressed appropriately; and Effectively communicate the hospitals/clinics/ambulance in emergency or illness situations.

Medical Education- Provide education to clients on conditions, medications, hygiene, sexual diseases, and other topics as needed or requested; Provide education to client parents/guardians about medical condition, changes, and medication management; and Effectively communicate with parents/guardians related to any concerns.

Reporting/Documentation- Complete and transcribe psychiatrist and consulting physicians' orders as needed; Prepare medical records for physicians; Obtain consents and other date as needed; Audit and update medical records including MAR's and medication errors; Ensure required documentation is provided as required; Complete medical file documentation for services as needed; Maintain client files ensuring updates and accuracy; and Provide records in accordance with COA/HIPAA.

Other- Participate in meetings as appropriate; Assist with medical record utilization review as needed; Serve on rotation on-call schedule; and Other duties as assigned.

EXPERIENCE/EDUCATION/LICENSES/CERTIFICATIONS:

  • Must be 21 years of age required
  • Associate degree in Nursing required
  • Licensure as Practical Nurse required
  • Must successfully pass Fit-For Duty Health Screen prior to start of employment required
  • 1 year of psychiatric nursing experience preferred
  • 1year working with youth in behavioral health preferred
  • Certification in CPR, First Aid, CPI, and other Agency training provided with 90 days of hire required
  • Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges.
  • Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
  • Valid driver's license required

Work Environment:

This job operates in a professional office environment and in the community. This role routinely uses standard office equipment such as desktop and/or laptop computers, photocopiers, and smartphones. This individual also will need to be able to lift 50 pounds in this role
