Job Description

Position Overview

The Patient-Centered Medical Home (PCMH) Coordinator is responsible for the day-to-day development and implementation of the NYS- PCMH Model at UHS. The Patient-Centered Medical Home (PCMH) is a model of care that puts patients at the forefront of care. PCMH is a care delivery model where the primary care team, along with the patient (and family, when appropriate), are responsible for managing the full spectrum of health care needs. The PCMH model focuses on care coordination, population health, evidence-based guidelines and effective use of HIT to meet patients’ needs. NYS PCMH recognition helps practices put the structure, systems and processes in place to be effective medical homes. The PCMH Coordinator designs, initiates and facilitates NYS-PCMH workflow and processes in the UHS, UMA, CMH, and DVH Primary Care Practices. Duties include but are not limited to performance monitoring, data collection/validation/auditing, annual accreditation submissions system wide, staff/provider education, policy and procedure revision/updates and tracking and implementation of workflow changes in accordance with PCMH requirements. The Patient-Centered Medical Home (PCMH) Coordinator will serve as a resource and subject matter expert on NCQA and NYS-PCMH requirements and regulations.

$22.97 - $34.46 per hour, depending on experience

Education/Experience

Minimum Requirements:

  • High School or Equivalent
  • 3 years’ experience in a health care setting

Preferred:

  • Advanced Degree
  • Previous experience working in a Walk-in/Primary Care/Specialty Care Setting
  • Previous Leadership/Supervisory experience
  • Previous Performance improvement experience and/or experience providing education to staff.
  • Knowledge of NCQA/PCMH, Joint Commission, NYS Department of Health, CMS and Article 28 standards.
  • Working knowledge of Microsoft Word, Excel and power point programs

License/Certification

Minimum Required:

  • NCQA (National Committee of Quality Assurance) PCMH-CCE (Patient-Centered Medical Home, Certified Content Expert)
    • Completion of the PCMH Certified Content Expert (CCE) within 6 months of hire. Certification lasts two years. During that two-year period, CCEs must earn a total of 30 Continuing Education Unit (CEU) points.
