- Communicates in person or remotely, via email, phone, digital message service, video chat, etc. to assist clients in completing applications for program eligibility from a client centered approach with attention to detail and accuracy.
- Collects and reviews required documentation submitted in person, or digitally for accuracy, completion and compliance with program requirements, division protocols and workflow systems.
- Conducts home visits to retrieve or assist clients with required documentation and other related application assistance.
- Maintains, organizes, and uploads client documents and records electronically including but not limited to the case management software system, while adhering to division workflow protocols regarding the confidentiality and maintenance of client records in a timely and organized manner.
- Assists with coverage for front desk duties as needed and assigned, answering phones and responding to the public inquiries in alignment with division protocols and expectations for coverage.
- Prepares reports regarding eligibility determination results, protocols, related projects and peer reviews in a timely manner as assigned.
- Interacts with clients to provide information about application procedures and basic division programs available to clients from a client centered approach.
- Coordinates with internal/external stakeholders regarding collected documents, applications, client records, unique circumstances, or other internal/external related projects as assigned.
- Participates in outreach activities in the community as assigned.
- Performs other duties as assigned.
- Participates in activities and duties related to emergency management during a local state of disaster as directed by appropriate county managers.
MINIMUM JOB REQUIREMENTS:
- High school diploma/GED and some college level course work in Social Work, Psychology, or Counseling. Bachelor's degree in Social Work, Psychology, Counseling or a related field of study preferred Licensed BSW preferred.
- 1-year job related experience.
- Experience working with/in eligibility, advocacy, navigation, or wrap around services in a social service setting with special populations that include, victims of crime, seniors, children/adolescents, mental health, hospital, homeless, unstably housed, nursing home, schools, advocacy, or agency setting.
- Experience working with Caseworthy, HMIS, or other Case Management Software, and grant funded programming. Experience with SNAP, SSI, SSDI, WIC, and other local, state and federal benefits application based programs.
- Strong written and verbal communication skills. Strong interpersonal skills. Ability to work well on a team to accomplish goals. Ability to work on multiple projects with clear deadlines in a timely manner. Ability and willingness to work with a diverse range of groups, including all communities, ethnicities, religions, and cultural and social groups/backgrounds with a client-centered approach.
- Ability and willingness to travel into the field and participate in outreach events as needed. Ability to work independently, be resourceful and take initiative. Strong computer and software skill sets, including Microsoft Office suite. Strong organizational, collaborative, and innovation skill sets. Strong time management skills. Ability to prioritize tasks and duties and be flexible. Strong problem solving and de-escalation skills. Attention to detail and accuracy. High level of client satisfaction standards.
STARTING SALARY RANGE: $17.95 - $22.44 hourly based on qualifications
CLOSING DATE: Upon filling position
