Description

Nexus Family Healing is looking to hire a Nursing Supervisor to join our newest youth crisis and stabilization center set to open in fall 2025 in Hennepin County!

This new youth crisis and stabilization center is a 13-bed, short-term residential service is for Hennepin County youth with complex mental and behavioral health issues looking for appropriate care. The service will help to reduce the number of youth who are stuck in inappropriate care settings such as emergency rooms, police stations, juvenile detention or unstable home situations and provide treatment and therapeutic supports while triage, assessment, and transition planning take place with families.

Pay and Schedule:

  • Full-time onsite opportunity
  • Location: 1800 Chicago Avenue in Minneapolis
  • Salary Range: $90,000-$105,000 annually

Position Summary: The Nursing Supervisor monitors the provision of medical services that promotes resident physical health and well-being and ensures related staff training is provided per licensing guidelines.

Primary responsibilities:

Medical Service Delivery

  • Coordinates medical aspects of admissions and ensures completion of annual physical exams; arranges necessary follow-up appointments and/or referrals.
  • Coordinates the nursing department's effective delivery of medical services to the resident population while partnering closely with the program team.
  • Coordinates special needs appointments with external physicians and clinics.
  • Collects and assesses residents' medical data in a systematic and ongoing basis, ensuring the medical records are always current.
  • Monitors acute and chronic medical conditions of residents; provides treatment or consultation, as needed and communicates with physicians with appropriate.
  • Gathers relevant information and provide appropriate care in emergency situations. Directs others as necessary.

Treatment Team Collaboration

  • Serves as a resource to staff on an ongoing basis regarding resident medical issues.
  • Participate in new client diagnostic meetings to ensure that a cohesive individual diagnosis for each client is recognized in the client's ITP.

Implement, monitor, and ensure the compliance of all medical service documentation.

  • Reviews and assesses Medication Administration Records (MAR's) on a regular basis.
  • Supervises the provision of medication administration and documentation; reports any side effects of medication to physicians; reports documentation errors/omissions to appropriate staff management.
  • Coordinates and/or provides staff training in medication administration and infection control.
  • Monitors prescription and medical supplies for all programs and ensures orders are placed in a timely fashion.
  • Provides and ensures proper tracking of staff Hepatitis vaccines per contracting agency guidelines.

Provides first aid care and/or consultation to employees regarding work-related injuries as needed.

Employee and Talent Selection, Engagement and Performance Management

  • Supervises and participates in the formal process to orientate, mentor, train and develop staff within Nexus and licensing guidelines and ensures the process is consistently and effectively administered.
  • Leads, coaches and develops staff and provides skills training and education regarding resident issues, individual resident's treatment plans and phases, the provisioning of direct services, behavior management techniques, compliance regarding Medicaid standards and the preparation and maintenance of resident case files and documentation.
  • Provides performance management for staff on an ongoing basis and conducts formal performance evaluations that are timely and constructive, that include employee development opportunities.
  • Consults with the Human Resources Department on employee performance issues and opportunities.
  • Demonstrates the ability to develop a cohesive and effective team, through the use of basic supervision and teambuilding components such as; communicating a clear mission to the team, outlining common goals and promoting shared accountability for outcomes; identifies problems that can inhibit team/staff development and chooses facilitation strategies to help achieve team goals.
  • Demonstrates the knowledge and understanding of all policies and procedures and enforces policies while holding staff accountable.

Nursing Program and Internship Facilitation and Coordination

  • Sources and recruits top talent from local colleges and universities.
  • Develops an on-site curriculum for the Nurse rotational program.
  • Supervises and facilitates continuous education for internship program.
  • Provides feedback and updates to colleges regarding the Nurse rotational program.

Requirements

Required Education and Licensure:

  • Graduate of an accredited school of nursing
  • Valid Registered Nurse license
  • Minimum of 5 years related experience; preferably in a residential setting
  • First Aid/CPR/CPI/Infection Control certification.
  • Valid driver's license required. Must meet state regulating agency and Home Office driving requirements

Preferred Education and Experience:

  • 2 years of supervisory experience.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision and ability to adjust focus. The job requires extended periods of sitting. May need to be able to assist staff in proper restraining of residents. Must be able to lift up to

50 pounds unassisted and up to 100 pounds with assistance. Must be able to twist, bend and stretch in a manner conducive to the execution of daily activities within a residential treatment program and office.
