Seeking a compassionate and skilled Part-Time Home Healthcare RN Case Manager to join our team in Fort Collins, Colorado. If you're passionate about providing exceptional care to patients in the comfort of their homes, this role is for you.
Requirements:
- Registered Nurse (RN) license in the state of Colorado
- Experience in home care nursing preferred
- Strong communication and organizational skills
Responsibilities:
- Assess patient needs and develop individualized care plans
- Coordinate care with other healthcare professionals and agencies
- Provide direct patient care as needed
- Educate patients and families on treatment plans and self-care techniques
Compensation:
- Competitive pay rate: $38.00 - $45.00 per hour, based on experience
- Mileage reimbursement
