Job Overview:
Day to day, you would be responsible for providing direct oversight of bed placement and transfer center, under the direction of the DCARE Manager. The Access and Logistics Team coordinates patients throughout from all points of entry to facilitate overall hospital patient flow. This team receives all communication related to patients that are requiring bed placement for appropriate clinical expertise and facility placement to support the delivery of quality care. The Patient Placement Team collaborate with hospital staff, ADCO, physicians, staff, Resource Team, and Transfer Center to define, reduce, and eliminate barriers within the system which may impede patient flow and surge plan. These employees also coordinate nursing home triage calls as needed.
Required: Certifications/Licenses/Experience:
- Active Registered Nurse (RN) in Indiana or other compact licensure state
- A minimum of three years of clinical nursing experience in an acute care setting or related specialty area
Preferred Certification/License/Experience:
- BSN
- Previous leadership experience
