New Age Hospice is seeking a Hospice Licensed Social Worker-PRN in the Houston, TX and surrounding areas.

Job Duties
- Territory includes, Houston and surrounding areas
- Provide individual and family counseling in home
- Complete relevant documentation
- Consult with hospice team (IDT)
- Participate in IDT
- Develop care plans
- Provide resources to assist patient and families in the community
- Provide PASSR information and coordinate respite care
- Application assistance to Medicaid and other insurance providers
- Provide case management, advocacy, resource linkage as needed
- Provide visits as directed to include admission, recertification, and care plan necessitated
- Maintain professional relationship with staff and referral sources
Requirements
- Master's degree in social work and associated licensure - MSW
- 1 year experience preferred working as social worker with hospice or home health care.
- Ability to work in a professional manner
- Must work well both independently and in a group setting
