We are hiring a Social Services Professional to support the needs of our incredible Adult Day Health Program! We are offering competitive pay and benefits including Medical, Dental, Vision, generous PTO, and much more!!
The hourly range for this role is $25.63-26.63 DOE. We look forward to talking with you about this incredible opportunity!!
Our Support Service Professionals:
·Provides information to referral sources and potential clients by receiving intake inquiries and giving tours;
·Identifies clients’ service needs by gathering and completing intake information and working with the client, caregiver and/or referral source to develop service plans;
·Maintain accurate records on housing search process for verifiable billing. Document and communicate with clients, agency and community partners on housing search progress.
·Creates and attend networking events, industry conferences, and community gatherings to expand your network.
·Trains new team members as they gain experience to support the sales effort.
·Reinforces the community’s brand reputation and achieve maximum occupancy goal.
·Develop a person-centered plan and assessment to meet the needs of community integration to include short and long-term goals as well as goal-driven action plans to address client needs and concerns.
·Provide case management and create individualized care plans to meet the needs of our clients
What you will need to succeed!
·Master’s degree in social work, gerontology, or other human services field, or counseling and at least one year of professional work experience (full-time equivalent), or a bachelor's degree in social work, counseling, or a related field and two years of experience in a human services field, or related field.
·Must complete the HIV/AIDS information course;
·Must receive an annual influenza vaccine unless granted a medical or religious exemption.
Physical Requirements for the role:
·Must be able to lift 25 lbs. and support clients’ mobility;
