Fraser is currently hiring a Designated House Coordinator to join our Community Living team. As a DHC, you will provide site-specific supervision and leadership to assure continuous quality improvement in active treatment service development and implantation at one of our Supervised or Supportive Living locations.
Requirements:
Candidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.
- HS Diploma or Equivalent AND four (4) years of full-time work experience (8,000 hours) providing direct care services to persons with disabilities or nursing/assisted living home experience
- BA/BS in Human Services or related field AND 1 year of full-time work experience providing direct care services to persons with disabilities
- Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities
Responsibilities:
- Manage staff coverage to meet the needs of the clients served
- Coordinate completion of all household repairs and other upkeep
- Coordinate the activities of daily living including personal care, hygiene, and grooming
- Provide direct cares to our clients
- Assure documentation for all areas of client care are completed
- Collaborate with medical professionals to coordinate health and medical needs for each client
- Coordinate weekly, meal planning, and nutrition for clients

This position is full time Monday-Friday, standard business hours
The starting pay range for this role is $21.50 - $23.50 per hour depending on up qualifications
