Tru Medical is seeking a compassionate and motivated Health Coach to join our care team. In this role, you’ll support a panel of patients in achieving their health-related SMART goals through personalized education, motivation, and collaboration with their healthcare providers.


Key Responsibilities

  • Support patients between visits by reviewing care plans, providing education, and identifying treatment barriers to achieve or maintain control of their chronic conditions like Hypertension, Diabetes, Chronic Kidney Disease etc.
  • Enroll patients in the Health Coaching Program and explain how the program supports their care.
  • Educate patients on chronic conditions such as diabetes, hypertension, asthma, and more using approved materials.
  • Use motivational interviewing to help patients set and track SMART health goals.
  • Develop and update care plans in the EMR (eClinicalWorks).
  • Monitor progress and adjust goals based on patient outcomes.
  • Collaborate with providers and care teams through regular meetings.
  • Encourage follow-up appointments and adherence to self-care routines.
  • Participate in ongoing training and report on program metrics.


Experience/qualifications:

  • 1+ year in a similar role preferred (e.g., Medical Assistants, LPNs)
  • No experience required with a completed Medical Doctorate (international degrees accepted)
Education:
  • Medical Assistant certification preferred
  • Completion of a Health Coach program preferred
  • High school diploma or equivalent required
Skills:
  • Knowledge of chronic disease management
  • Motivational interviewing skills
  • Proficient with computers and data entry
  • Bilingual in Spanish strongly preferred
  • Strong communication and interpersonal skills
  • Team player with a growth mindset

Salary
  • $20-25/hr plus performance-based bonuses




Schedule

  • 8-hour shifts (8:50am-5pm), full-time or part-time
  • Must be able to reliably commute or relocate to our office in Brooklyn, NY 11206

Compensation: $20.00 - $25.00 per hour
