As a Home Health RN Case Manager, you'll manage your own patient caseload, building lasting relationships while ensuring care plans support each patient's health goals.

What You'll Do:
Perform patient assessments and develop care plans tailored to individual needs.
Coordinate care and communicate with physicians, families, and your care team.
Monitor patient progress and adjust plans to support the best outcomes.
Provide education and support to patients and families for confident home care.
Enjoy a balance of field visits and collaboration with your team.
Requirements:
RN license in Ohio and CPR certification.
1-2 years of nursing experience (home health preferred).
Strong communication and organizational skills.
Passion for helping patients live well at home.

Starting pay for RNs with 1 year of experience start at $31.57/hour.
