What You'll Do:

  • Assessthe psychosocial status of patients
  • Providecommunity and governmental resources
  • Conductsocial evaluations and plan interventions
  • Developindividualized care plans
  • Assistphysicians with social and emotional factors
  • Participatein quality assessment performance improvement
  • Liaisebetween patients and community agencies
  • Maintainand develop contracts with public and private agencies
  • Assistwith bereavement coordination

Qualifications:

  • Accredited master’s degree in social work
  • Minimum one year of healthcare experience
  • Home health or hospice experience preferred
  • Current CPR Certification
  • Licensed driver with an insured automobile
