What You'll Do:
- Assessthe psychosocial status of patients
- Providecommunity and governmental resources
- Conductsocial evaluations and plan interventions
- Developindividualized care plans
- Assistphysicians with social and emotional factors
- Participatein quality assessment performance improvement
- Liaisebetween patients and community agencies
- Maintainand develop contracts with public and private agencies
- Assistwith bereavement coordination
Qualifications:
- Accredited master’s degree in social work
- Minimum one year of healthcare experience
- Home health or hospice experience preferred
- Current CPR Certification
- Licensed driver with an insured automobile
