Join a great team! DMC Primary Care, a physician-owned, independent practice, is seeking a full-time experiencedFamilyNurse Practitioner to join our growing team of primary care providers as a Home Visit APP.
Position Overview:
The Home Visit Advanced Practice Provider (APP) provides high quality, cost-effective Primary Care to homebound high-risk patients in New Hampshire. This role incorporates complete primary care services in the home to include chronic disease management, transitions of care, goals of care, annual wellness visits, and acute care needs. The goal is to provide patient-centered care to improve patient outcomes and reduce unnecessary utilizations.Position Responsibilities (Include but are not limited to):
- Performing comprehensive in-home assessments for home limited/high risk patients.
- Diagnosing acute and chronic conditions, order and interpret diagnostic tests (labs, imaging, etc.)
- Developing evidence-based treatment plans and prescribe medications within State scope of practice
- Coordinating/Collaborating with care team, providers, specialists, and community agencies
- Conducting serious Illness conversations including: POLST, Advance Care Planning, Portable DNR, Goals of Care
- Providing acute care interventions as appropriate
- Managing ongoing chronic disease care (CHF, COPD, CKD, etc.)
- Providing health coaching and patient education on medication adherence, lifestyle changes, and disease prevention.
- Ensuring accurate clinical documentation of diagnosis to provide exceptional evidence-based care.
- Offers flexible schedule

- Completion of an accredited graduate-level Advanced Practice Provider(APRN or PA) program
- National Certification and New Hampshire State Licensure as a NursePractitioner or Physician Assistant
- A minimum of three years of previous outpatient primary care experience is preferred
