Job Summary:
The Helping Others Provide Encouragement (HOPE) MDT Navigator is dedicated to supporting and coordinating services for youth and families with behavioral health needs, ensuring that wraparound principles are implemented and sustained. In this position, you will work closely with multidisciplinary partners to monitor service delivery, drive referrals, facilitate meetings, and track progress through Care Manager. Acting as a liaison between families, providers, and state agencies, you will ensure culturally competent, trauma-informed, and strengths-based care is provided.

Key Responsibilities:
- Build and maintain relationships with youth, families, and multidisciplinary partners, including state agencies, courts, schools, law enforcement, healthcare providers, and community organizations.
- Coordinate and monitor referrals, appointments, treatment goals, and services to ensure timely, effective care aligned with wraparound principles.
- Facilitate and participate in multidisciplinary team meetings, family support team meetings, and statewide collaborative sessions.
- Document and track service delivery, communications, and measurable outcomes in Care Manager in a timely manner.
- Engage families and natural support in planning, ensuring their voice and choice are central to decision-making.
- Identify and resolve barriers to service coordination between families, providers, and stakeholders.
- Apply trauma-informed, culturally competent, and strengths-based approaches in all interactions.
- Participate in professional development, training, and quarterly collaborative sessions with peers and partner agencies.
- Maintain up-to-date knowledge of behavioral health systems, resources, and community-based services for accurate guidance and referrals.
- Perform all other duties as assigned.
Education, Experience, and/or Credential Qualifications:
- Qualified Mental Health Professional (QMHP) or Qualified Addiction Professional (QAP) credential.
- Bachelor's degree in human services (e.g., social work, psychology, nursing, education, criminal justice, counseling, child development, human services, behavioral science) or related field.
- Any four-year combination of higher education and qualifying experience.
- Associate of Applied Science in Behavioral Health Support from an approved institution.
- Four (4) years of qualifying experience delivering services to individuals with mental illness, substance use disorders, or developmental disabilities.
Additional Qualifications:
- Preferred experience working in multidisciplinary teams and community-based behavioral health settings.
- Current driver's license, acceptable driving record and current auto insurance.
- Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.
- Commitment to service excellence, personal integrity, and continuous professional growth.
Physical Requirements:
- ADA Consideration- Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
- Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
