Hospice Medical Social Worker (LMSW or LLMSW/LiMSW)

Your Responsibilities:

  • Conducts initial and ongoing hospice psychosocial assessments for patients and families.

  • Collaborates with the interdisciplinary hospice team to develop, implement, and update the Plan of Care.

  • Provides skilled social work interventions in accordance with physician orders, professional standards, and hospice regulations.

  • Communicates changes in patient condition, safety concerns, and care needs to leadership and care team members.

  • Supports patients and families with counseling, education, and access to community and financial resources.

  • Maintains accurate, timely documentation and ensures compliance with CMS, Medicare, and payer requirements.

  • Implements strategies to promote patient safety and reduce avoidable hospitalizations or ER visits.

  • Upholds Elara Caring's mission while maintaining strict HIPAA privacy and confidentiality standards.

What You'll Bring

  • Master's or Doctoral degree in Social Work from a CSWE-accredited program

  • Current LMSW or LLMSW/LiMSW, per state licensure requirements

  • Minimum of 1 year of social work experience in a healthcare setting (hospice experience preferred)

  • Strong assessment, counseling, advocacy, and care coordination skills aligned with professional social work standards

  • Ability to travel locally (approximately 50% field-based travel)

  • Valid driver's license, reliable transportation, and current auto insurance
