The Hospice Social Worker provides medical social services to patients receiving end of life services.The Hospice Social Worker provides assessments and a plan of care considering the unique bio-psychosocial needs of the patient and family. The Hospice Social Worker provides education, support, individual and family counseling, linkage to community resources/services, and ongoing collaboration with the IDG and community health providers

Additional Job Description

Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners.Certification in case management preferred.
