Summary of Duties

As a Human Services Case Manager you will:

Determine initial and on-going Medicaid eligibility for long term services and supports (LTSS), to develop and implement service plans, to coordinate the provision of services for community-based care and nursing homes, and to regularly review and update Medicaid eligibility and monitor service plans.

Major duties include:

Engage in effective team participation through willingness to assist and support co-workers, supervisors, and other work-related associations. Develop positive working relationships with agency staff and managers through active participation in accomplishing group projects and in identifying and resolving problems in a constructive manner. Demonstrate openness to constructive feedback and suggestions, in an effort to strengthen work performance. Actively engage in and contribute to a positive, respectful, and productive workplace culture and atmosphere.
  • Screen and interview applicants/recipients, relatives, and other interested persons in the field, to determine initial and on-going eligibility for Long Term Care Services, health care, social service, diversion/transition, and protective service's needs. Inform clients of rights and methods of recourse.
  • Develop detailed, individualized case management plans for clients. Conduct on site field reviews using OR ACCESS, CA/PS assessment tool and the laptop computer. Provide on-going case management by coordinating the provision of services to clients according to rules, regulations, and the individual case plans. Conduct (direct and indirect) contacts with consumers or their authorized representatives.

Minimum Qualifications

A valid driver's license and acceptable driving record are required for this position.

A bachelor's degree in a Behavioral Science, Social Science, or a closely related field;

OR

A bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);

OR

An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);

OR

Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

Essential Attributes

  • Experience effectively working independently, using problem solving and critical thinking skills
  • Experience analyzing information or situations to make good decisions within program rules and policies
  • Professional communication skills required to interact with consumers, community partner and co-workers
  • Experience preparing and maintaining written narrative documentation of actions taken
  • Experience interviewing to obtain information to evaluate the next steps to take in a process

Working Conditions

  • Your work schedule will be Monday- Friday 8:00am-5:00pm
  • Travel is required to meet business needs and to attend employee training classes.
  • General office environment. You will visit clients, hospitals, nursing homes, APD (Aging and People with Disabilities) offices and other providers.
  • May work with individuals experiencing trauma or crisis. Some individuals may have challenges managing their emotions.
  • May encounter situations requiring quick decision-making to ensure the safety of oneself or others.
  • Employees in these positions may be required to work a flexible work schedule as a condition of employment. This is understood to mean that the hours of work may vary from day to day and could include evenings and weekends under certain circumstances. Work schedule specifics will be determined by the hiring supervisor.
  • Fast paced environment with constantly evolving policies and procedures
  • May drive a variety of different types of vehicles to remote places in a variety of terrain and possibly inclement weather.
  • Will come in contact with animals.
  • May be exposed to communicable diseases, such as flu virus, Covid-19, and MRSA.
