POSITION:                           LICENSED PRACTICAL NURSEDEPARTMENT:                  RURAL HEALTHFLSA STATUS:                    NON-EXEMPTPlease read this job description carefully to ensure that you understand its contents, the job requirements, and expectations before signing this document.JOB SUMMARY: Continuously responsible for providing professional nursing service (according to qualifications and training) and non-professional services as directed.  Directly responsible for performing patient care functions according to established standards, policies and procedures.JOB RELATIONSHIPS:            Responsible to the Rural Health Clinic Director.            Positions Directly Supervised:           None            Regular Contact with all Hospital departments, Physicians, and the public.MISSION STATEMENT:As an integral part of our community, Franklin Medical Center’s mission is to provide the highest quality health care and customer satisfaction to all those we serve. We are committed to delivering compassionate, capable, and personalized treatment to our patients and their families, always keeping in mind that our responsibility is to the health needs of the people.ESSENTIAL JOB FUNCTIONS:
  1. Obtain and record vital signs, assessments, chief complaints, medical history, height/weight, etc. as appropriate
  2. Perform basic clinical procedures such as wound dressing, specimen collection, point of care testing, EKGs, etc. (as trained)
  3. Administer and record all medications and treatments based on provider orders accurately using proper procedures
  4. Record all care information concisely, accurately, and completely in a timely manner
  5. Assist providers with exams, minor procedures, and urgent care needs
  6. Prepare, stock, and sterilize exam rooms and clinical equipment
  7. Communicate test instructions and follow-up plans to patients clearly and effectively
  8. Assist with patient outreach for lab results, chronic disease follow-up, and preventative-care reminders
  9. Collaborate with the care team to ensure timely documentation and quality care delivery
  10. Accurately document all patient interactions, procedures, medications, and communications in the Electronic Health Record
  11. Follow RHC regulations, safety protocols, and infection control standards
  12. Monitor and maintain clinic supplies and equipment usage. Notifies appropriate departments to replenish stock, as necessary.
  13. Participate in Performance Improvement on a monthly basis.
  14. Maintain knowledge regarding use of defibrillators, holter monitors, etc.
  15. Perform and documents quality controls daily
  16. Uphold HIPAA and patient privacy standards
  17. Provide basic health education, medication instructions, and lifestyle guidelines within skill level and training
  18. Maintains a neat and orderly work area.
  19. Assists with diabetic foot care, as needed
  20. Performs other duties as assigned.
QUALIFICATIONS:Education and Training: Graduate of a course for LPN. Must be licensed in the state of Louisiana.  CPR certified.Work Experience:   No experience necessary.Knowledge, Skills, and Abilities Required:
  • Excellent communication skills (oral and written)
  • Familiarity with electronic medical records (EMR)
  • Interpersonal skills and compassion
  • Analyzing information, critical thinking, and problem solving
  • Attention to detail to ensure accuracy
  • Basic computer skills
Physical Requirements:
  • Remain in a stationary position for extended periods of time.
  • Occasionally move about the office and facility to access various departments and patient care areas.
  • Constantly position self to maintain equipment and provide patient care.
  • Constantly operates a computer, office, and patient care machinery.
  • Ability to communicate information accurately so others will understand.
  • Ability to observe details at close range.
  • Occasionally assist patients such as lifting, pushing, or pulling, up to 50 lbs.
  • Frequently move boxes, equipment, etc. weighing up to 20 pounds across the office for various needs.
