Job Description

At Phoenix Home Care and Hospice, we provide unparalleled care to pediatric and young adult clients with diverse health needs.

We are currently seeking dedicated LPNs or RNs to join our team in our mission to provide exemplary care.

In this role, you will use your clinical skills and compassion to support children and young adults in receiving the family-centered care they deserve. Your work will involve creating meaningful connections with clients and their families, ensuring their health and comfort as they navigate various medical challenges.

Responsibilities

  • Develop, review, and revise the individualized plan of care for clients.
  • Administer nursing care in accordance with physician’s orders.
  • Monitor and document client’s progress and report any changes to appropriate personnel.
  • Educate clients and families regarding the plan of care and ongoing health management.
  • Utilize electronic charting for accurate and timely documentation.
  • Participate in team meetings and quality assurance activities.

Requirements

  • Active Licensed Practical Nurse or Registered Nurse License.
  • Strong problem-solving skills and ability to multitask.
  • Excellent communication skills and a positive demeanor.
  • Passion for providing quality healthcare to pediatric and young adult clients.
