The role includes supporting residents with daily clinical needs, performing assessments, administering medications, and collaborating with the interdisciplinary care team. The ideal candidate is dependable, patient-centered, and comfortable working in a fast-paced SNF environment with rotating responsibilities.
Key Responsibilities- Deliver safe and effective nursing care to SNF residents.
- Perform assessments, monitor conditions, and communicate changes to the RN or provider.
- Administer medications, treatments, and wound care as ordered.
- Assist with ADLs, mobility support, and resident comfort measures.
- Document all care accurately in the EMR (Cerner).
- Work collaboratively with nursing staff, physicians, and support teams.
- Maintain infection control and safety protocols.
- Provide support across assigned units as needed during 12-hour and 8-hour shifts.
- Active LPN license (ND license accepted upon offer).
- Minimum experience requirement: less than 1 year acceptable.
- SNF or long-term care experience preferred.
- Cerner EMR experience preferred.
- Strong clinical judgment and time-management abilities.
- Ability to work 12-hour and 8-hour rotations, including holiday coverage.
