Job Description
Identifies and implements community health improvement (CHI) initiatives that address social needs, and social determinants of health in collaboration with hospitals, shared services, and community groups in alignment with Saint Luke's BJC Health System CHI strategy and priorities.
Develops and maintains relationships with various community organizations.
Seeks opportunities to pursue participation in research, planning and/or the evaluation of program components. Tracks, analyzes and reports on relevant data and disseminates findings appropriately. Implements findings and uses data to improve care of patients and the systems of healthcare delivery.
Supervises project managers.
Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
Collaborates with hospitals, shared services, and community groups to effectively implement community health improvement initiatives.
Stays current on community health improvement practices, research, and technical skills.
Develops and adheres to departmental staffing, revenue and/or expense budgets. Responds to changes in the business which may affects the ability to achieve the budget goals.
Experience in conducting Community Health Needs Assessments (CHNA) and developing Community Health Improvement Plans (CHIP) in accordance with IRS regulations.
Requires 5-10 years of experience and 2-5 years of supervisor experience.
