Military Case Manager 1 (Full-Time)

Position Summary: To provide direct and indirect patient care and case management services to active-duty military patients receiving inpatient care for substance abuse, mood disorders and/or posttraumatic stress disorder.

*Pay Range: $22/hr. to $25/hr.

Essential Duties:

• Manage multiple cases from admit to discharge
• Command and Provider communication
• Work within a multidisciplinary clinical team
• Write brief reports and clinical updates
• Take care of patient needs during their treatment
• Discharge planning
• Attend clinical treatment teams
• Administrative: faxing, filing, report preparation

Requirements

Education:

• Bachelor's degree in Psychology or Social Work or related clinical field

Experience:

• A minimum of one year work experience pre or post-graduation
• Interest in the field of Behavioral Health
• Strong oral and written communication skills
• Ability to work effectively both independently and as part of an interdisciplinary treatment team

Desired Qualifications:

• Professional experience working with military families or personal military lifestyle experience
• Knowledge of the difficulties of dealing with the requirements of military life
• Knowledge of military and community resources appropriate for patients
