A brief overview of Responsibilities:

  • Conduct comprehensive psychosocial assessments within 5 days of admission
  • Assess and address emotional, social, financial, and environmental needs, identifying appropriate interventions
  • Participate in Interdisciplinary Group (IDG) meetings to develop and revise individualized plans of care
  • Obtain physician orders in accordance with patient needs and agency protocols
  • Monitor, document, and communicate changes in patient and family psychosocial status
  • Maintain complete and accurate clinical records, including assessments, care plans, and progress notes
  • Evaluate the effectiveness of social work interventions and adjust care plans as needed
  • Coordinate weekly visit schedules in collaboration with Case Managers
  • Participate in Quality Assessment and Performance Improvement (QAPI) initiatives
  • Provide ongoing support to the IDG, even if social work services are initially declined by the patient or family
  • Assist patients and families with coping strategies and connecting to community resources
  • Perform additional responsibilities as assigned by agency leadership

Qualifications:

  • Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education

  • At least one year of Hospice experience is preferred.

  • The ability to communicate well, both verbally and in writing.
