Job Description
Position Title: Nurse Practitioner / Physician Assistant – Primary Care
Location: Prescott, Arizona
Employment Type: Full-Time
Schedule: Monday – Friday (No weekends, no call)
Clinical Setting: Outpatient Family Medicine
Travel Requirements: None
Average Daily Patient Volume: 15–20
Compensation & Benefits
Annual Salary: $140,000 – $160,000 (commensurate with experience)
Bonus Structure: Percent of collections

Position Summary
We are seeking a skilled Nurse Practitioner or Physician Assistant to join our outpatient primary care clinic in Prescott. In this role, you will provide direct care to patients across the lifespan, managing both acute and chronic conditions. You’ll be part of a supportive clinical team focused on continuity of care, patient education, and improving outcomes across our community.
Key Responsibilities
- Conduct comprehensive physical exams and evaluate diagnostic tests
- Diagnose and treat acute conditions (e.g., infections, minor injuries)
- Manage chronic illnesses such as diabetes, hypertension, heart disease, and obesity
- Develop individualized treatment plans based on clinical findings
- Deliver patient-centered care that considers physical, emotional, and social health
- Coordinate with physicians, nurses, and specialists for integrated care
- Educate patients and families about conditions, treatment options, and wellness strategies
- Facilitate referrals, follow-ups, and hospital admissions when necessary
- Ensure accurate and timely documentation in compliance with standards
- Lead wellness screenings and preventive health initiatives
- Maintain up-to-date clinical knowledge through ongoing education
Required Qualifications
- Active Nurse Practitioner or Physician Assistant license in Arizona (or in progress)
- Graduation from an accredited U.S.-based NP or PA program (excludes Puerto Rico)
- Current or eligible DEA license
- BLS certification (ACLS preferred)
- Minimum 1 year of experience in Primary Care preferred
