Job Description
M-F 8-4:30 One weekend on-call/month
Job Summary
Responsibilities: Coordinates and facilitates Telehealth services and assists with data extraction and collection for key nursing quality indicators.
Essential Job Elements
Works with Director of Nursing Excellence and Quality and other members of nursing leadership to assist in the planning, design, implementation, and maintenance of a comprehensive nursing quality improvement program.
Assists Director of Nursing Excellence and Quality in identifying and problem-solving quality improvement issues related to nursing quality indicators and regulatory compliance.
Ensures compliance with quality improvement initiatives. Reviews records and documentation as needed for abstraction.
Gathers data and prepares reports that are needed for submission of quality data to various regulatory and accrediting agencies and internal data requests.
Provide support as needed by working closely with the IT team in the optimization of electronic medical record systems and related clinical tools to ensure system capabilities are in line with clinical reporting expectations. Utilize multiple software applications to prepare spreadsheets, graphs, charts and power point presentations.
Exhibits excellent communication skills and ability to interact with a diverse population. Effectively interacts and communicates with team members as well as external customers/clients/potential employees. Strong team working and collaborative skills.
Coordinates with external partners in telehealth services to schedule and facilitate telehealth visits with patients.
Navigates the Electronic Medical Record to assist providers as necessary with data collection for telehealth visits
Manages the patient follow-up appointment process as determined by the telehealth provider
Qualifications:
ADN required, BSN preferred. Prior experience in quality, regulatory, and/or compliance preferred.
