We are seeking a compassionate and skilled Occupational Therapist to provide in-home therapy services to patients in Macomb County, MI. In this role, you will help patients of all ages, including older adults, thrive safely and independently within their own homes. As a home health OT, you will have the autonomy to create personalized treatment plans, manage your own schedule, and provide 1:1 care in a meaningful way.

Key Responsibilities
- Conduct comprehensive in-home assessments of patients' functional abilities, including physical, cognitive, and psycho social skills.
- Develop and implement individualized, evidence-based treatment plans that promote patient independence and address specific functional limitations.
- Assess the home environment for safety hazards and make recommendations for home modifications and adaptive equipment.
- Educate and train patients, their families, and caregivers on therapeutic techniques, adaptive strategies, and the safe use of assistive devices.
- Collaborate with a multidisciplinary healthcare team, including nurses, physical therapists, and physicians, to ensure coordinated and effective patient care.
- Document patient evaluations, treatment progress, and discharge plans accurately and in a timely manner using our electronic system.
- Utilize clinical reasoning to adapt interventions to unique home settings and patient conditions.
- Manage your caseload efficiently while maintaining a high standard of clinical care.
Qualifications
- Graduate of an accredited Occupational Therapy program (Master's or Doctoral degree).
- Current, unrestricted OT license in Michigan.
- Current certification by the National Board for Certification in Occupational Therapy (NBCOT).
- Minimum 2 years of clinical experience, with home health experience preferred.
- Valid driver’s license, reliable transportation, and current auto insurance.
- Proficiency with technology and electronic medical record (EMR) systems.
- Strong interpersonal, communication, and problem-solving skills.
- Ability to work independently and manage your own schedule.
