CCAP Mission Statement - To empower all people and communities, challenged by poverty as well as social and cultural barriers, through advocacy, education, and access to high quality health and human services.

JOB SUMMARY:

The PAT Model has four components, each closely related and integrated: personal home visits, group connections, child screenings, and resource network. This program has the following primary goals: to increase parent knowledge of early childhood development and improve parenting practices, to provide early detection of developmental delays and health issues, to prevent child abuse and neglect, and to increase children's school readiness and school success. PAT program serves children and their families from pregnancy through kindergarten entry.

WORK SCHEDULE DEMANDS:

This is a full-time, 40 hours per week, hyrbird, home-based position.

Due to the nature of the work and level of responsibility, this individual maintains a flexible schedule, which may go beyond a regular workweek.

Requirements

REQUIRED QUALIFICATIONS:

  • BA/BSW or related in Human Services, Child Development, Education.
  • Minimum 3 years of public health or community development experience working with young children and/ or parents.
  • Complete all required background checks
  • Provide all required immunizations/vaccinations
  • Valid Driver's license and proof of active vehicle insurance
  • Bilingual Spanish and English; preferred

KEY RESPONSIBILITIES:

  • The PAT model is a voluntary, evidence-based home visiting model designed to ensure young children are healthy, safe, and ready to learn.
  • Parent Educators must attend PAT Foundational and Model Implementation trainings. In addition to the specific PAT trainings, staff is required to attend Life Skills Progression training and HEALTH's MIECHV (Maternal, Infant and Early Child Home Visiting) trainings.
  • This curriculum is utilized in the delivery of PAT services with families.
  • Meet with families on a regular basis in accordance with PAT Model standards per caseload capacity.
  • Attend reflective supervision and regular staff meetings.
  • Maintain boundaries with families in an ethical and professional manner.
  • Adhere to child abuse and neglect reporting laws.
  • Perform screenings for developmental milestones per recommended intervals.
