About the Position

We're looking for a Patient Care Manager to lead and develop our front-line team dedicated to supporting patients. In this pivotal role, you'll ensure every patient interaction—whether by phone, portal, scheduling system, or virtual visit—is handled with excellence, timeliness, and empathy. Reporting directly to the Head of Operations, you'll be instrumental in shaping the patient experience from first contact through ongoing care.

As Patient Care Manager, you'll guide a team of front desk specialists while collaborating with leadership to maintain service excellence, implement process enhancements that improve patient readiness and operational efficiency, and leverage data insights to drive strategic improvements. This is a full-time, remote position.

What You'll Do

Lead Patient Care Coordination

Direct daily patient-facing operations including inbound/outbound calls, voicemail management, appointment scheduling and adjustments, patient portal communications, and virtual visit support

Maintain adherence to service level agreements, performance metrics, and quality standards across all patient touchpoints

Resolve patient access barriers and service issues, escalating complex situations according to established protocols

Ensure thoroughness and accuracy in documentation, task completion, and data entry throughout patient care workflows

Build and Develop Your Team

Lead and mentor Patient Care Coordinators (PCC) in a remote work environment through regular coaching, feedback, and performance development

Conduct ongoing one-on-one meetings focused on service quality, response times, and patient experience outcomes

Foster a culture of accountability, continuous improvement, and patient-centered service

Optimize Systems and Processes

Manage omnichannel communication tools and operational platforms (phone systems, patient portals, EMR, scheduling software) to support seamless patient care delivery

Identify inefficiencies in workflows, handoff procedures, and recurring service gaps, bringing forward actionable recommendations to leadership

Contribute to process documentation and improvement initiatives that enhance service delivery

Partner Across Functions

Collaborate with Clinical teams, Registered Dietitians, and Care Teams to ensure coordinated patient communications and scheduling

Work with B2B partners to facilitate referral intake processes and patient communication workflows

What We're Looking For

Experience & Education

Bachelor's degree or equivalent hands-on operational experience

Proven track record managing patient access, front desk, intake, or patient support teams in healthcare or telehealth settings

Deep familiarity with patient-facing workflows and service delivery standards

Skills & Capabilities

Track record of driving process improvements—documenting workflows, developing process maps, and identifying enhancement opportunities in collaboration with leadership

Strong ability to coach, develop, and support remote team members effectively

Proficiency with healthcare technology platforms including EMRs, scheduling systems, phone platforms, and operational dashboards

Exceptional organizational abilities with keen attention to detail

Strong technical aptitude and data-driven approach, comfortable analyzing metrics and adapting to evolving custom-built systems, EHRs, and communication tools (Slack, Google Workspace, etc.)

Excellent written and verbal communication skills with the ability to support team members and partner effectively across departments

Your performance will be evaluated based on:

Achievement of key operational metrics (call volume, portal response times, voicemail handling, internal task completion)

Timeliness and accuracy of scheduling and patient communications

Patient satisfaction scores and experience indicators

Team service quality, consistency, and responsiveness

Proper escalation of issues following established protocols
