Pay Range: $55.00 - $70.00/hr

Job Summary

The Physical Therapist (PT) is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

Responsibilities

• Provide physical therapy services to patients according to a written physician's plan of care
• Be responsible for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver
• Provide counseling to patient, caregiver, and family
• Provide education to patient and caregiver
• Prepare clinical notes and progress summaries based on the attainment of goals
• Communicate with the physician and other health care practitioners regarding the current home health plan of care
• Initiate physical therapy program and instructs other personnel and/or family/caregiver in certain phases of physical therapy with which they may work with a patient
• Instruct personnel and/or family/caregiver as to the goals of the physical therapy program for the patient by participating in case conferences
• Train patient in the use of prosthetic device
• Identify patient and family/caregiver needs for other home health services
• Participate in discharge planning for patient
• Provide in-service education programs for nursing organization personnel
• Participate in peer consultation process
• Supervise Physical Therapy Assistants

Requirements

• Active CA Physical Therapy License
• Strong verbal and written communication skills
• Strong organization skills
• Possesses and maintains current CPR certification
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
