Physical Therapist - Assisted Living Facilities

Responsibilities:- Provide physical therapy services to patients, including evaluation, treatment planning, and implementation of treatment plans.- Develop and implement individualized care plans based on patient needs and goals.- Monitor and document patient progress, adjusting treatment plans as necessary.- Collaborate with other healthcare professionals to ensure comprehensive patient care.- Educate patients and their families on exercises and techniques for at-home care.- Stay up-to-date with advancements in physical therapy techniques and research.

Requirements:- Valid license to practice as a Physical Therapist.- Experience in Home Health is preferred.- Knowledge of medical terminology, anatomy, and physiology.- Strong interpersonal and communication skills to effectively interact with patients and healthcare team members.- Ability to assess patient needs and develop appropriate treatment plans.- Compassionate and patient-centered approach to care.
