What You'll Do

• Deliver personalized physical therapy services to post-acute and long-term care residents
• Collaborate with a multidisciplinary team to develop and implement care plans
• Help patients improve mobility, strength, and overall physical function
• Document progress and maintain compliance with regulatory standards
• Educate patients and families on therapy goals and home exercises
