Job purposeTo providehigh qualityhealth care to thepatients of theFairHaven Community Health Centerintheprimary caresettingconsistentwith FHCHCpoliciesand mission. To focus, particularly, on high-needs elderly patients and other vulnerable adult populations.Duties and responsibilities
- Providesprimary medicalcareby caring for adult and geriatric (>60 years old) patients
- Develops a plan of careforeach patient,including medicalhistory,physicalexamination,diagnosisand appropriatetreatmentand/orreferral,including hospitalization ifnecessary
- Usesallavailableresourcesin diagnosisand treatment,such aslaboratory and otherstestsinan appropriateand costefficientmanner
- Conferswithotherclinicians,patient’sfamiliesandothersupportstaffconcerning treatmentand care of patientswhenappropriate
- Refersthosecaseswhich requirespecialistservices,butmaintainsprimarycareresponsibility to assurecontinuityofcare
- Maintainsaccuratemedicalrecordsand performsappropriatecharting and followup on patients
- ParticipatesinMedical & Generalstaffmeetingsand appropriateclinicalteams
- ParticipatesintheHealth Center’sQuality Improvementprogram,in other taskcommitteesasneeded,and pursuescontinuing educationopportunitiestoupdatemedicalknowledgeand assurequality patientcare
- Performs other necessarydutiesasrequired by theFHCHC to achievethegoalofprovidinghigh qualityserviceand health care
- Graduation froman accreditedmedicalschoolwith a degree of Doctor of Medicine or Doctor of Osteopathy.
- Completion of an approved internal medicine residencyprogram
- Completion of a geriatrics fellowship or equivalent work experience managing elderly patients
- Licensetopracticemedicinein State of Connecticut
- Qualified in BasicLifeSupportCPR
- Bi-lingualin Spanish and English is strongly desirable
- Experiencein an ambulatory or outpatientsetting preferred
- Abilitytorelatewithwarmth and effectiveness to thepatientsand other staff
