About AllCare.ai AllCare.ai delivers coordinated, in-facility care (Primary Care, Psychiatry, Podiatry, Diagnostics, Wound Care, CCM) with a 24/7 concierge. Our platform automates charting and compliance so you can focus on high-quality senior care.
Role Overview We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton. Immediate need: ~300 eligible residents are ready to be seen across our Stockton assisted living communities, providing steady, predictable volume.
What You’ll Do
- Routine & urgent assessments; chronic disease management
- Post-discharge follow-ups, med reconciliation, labs/diagnostics
- Coordinate with facility staff, specialists, families/POAs
- Same-day documentation; CMS/regulatory compliance
Schedule & Setting Part-time; weekday blocks preferred. Clustered Stockton routes to minimize travel.
Requirements
Qualifications
- Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
- Geriatrics or facility-based experience preferred; EMR proficiency
Benefits
• Compensation (CA pay transparency)• Annual compensation range: $150,000–$200,000 (based on experience, schedule, and patient volume)• Additional earning opportunities available based on coverage and panel needs• W-2 with benefits available; mileage reimbursed per policy• Final terms outlined in the employment agreement
