PRN OT – Covering Tri-County Area

As a Occupational Therapist, here’s a big-picture view of what you’ll do:
- Coverage for IDT meetings and In-Home / Clinic Assessments as needed.
- Conduct initial and periodic in-person assessments to evaluate participants’ functional abilities, home environment, and rehabilitation needs.
- Recommend and coordinate adaptive equipment, home modifications, and therapy services to promote safety and independence.
- Develop, implement, and revise individualized treatment plans based on participant progress and IDT collaboration.
- Deliver OT interventions across settings including the clinic, participants’ homes, and contracted facilities.
- Educate participants and caregivers on equipment use, home safety strategies, and ADL programs.
- Maintain timely, accurate documentation of assessments, treatment, and participant progress per regulatory and organizational standards.
- Actively participate in daily IDT meetings, contributing to comprehensive care planning and 24-hour care coordination.
- Supervise Certified Occupational Therapy Assistants (COTAs), including review of care plans, co-signing documentation, and providing ongoing mentorship.
- Train staff in safe OT practices and equipment use to enhance participant care.
- Support quality improvement, utilization review, and compliance initiatives.
- Perform other IDT duties as needed within scope of practice and competencies.
- Maintain current licensure, certifications, and participation in ongoing professional development.
- Maintain appropriate access and confidentiality regarding Personal Health Information (PHI), in accordance with organizational and departmental guidelines.

A few must-haves for our Occupational Therapist:
- Graduate with a degree in an occupational therapy program from a university, which meets, the educational and field work requirements of the American Occupational Therapy Association.
- Successful completion of the national examination for OTR and SC license required.
- 3 years OT experience in various settings preferred
- Experience with geriatric/Frail population
- Strong communication skills with the ability to collaborate effectively across all levels of clinical and non-clinical staff.
- Proven ability to multitask and prioritize responsibilities in a dynamic healthcare environment.
- Physically capable of assisting patients with mobility tasks such as standing, walking, sitting, and repositioning in bed.
- Competent in reading and interpreting technical instructions related to patient care plans and procedures.
- Skilled in observing and assessing patients through both visual and auditory cues.
- Adaptable and resilient in managing frequent changes in patient needs and care settings.
- Must provide a valid driver’s license and auto insurance
- Current CPR/BLS certification required
- Ability to pass drug Screening and backgrounds
- Up to date required immunizations (TDP, MMR, Varicella, FLU, and Pneumonia) and physical exam or statement of good health
- Must meet all applicable health and physical requirements necessary to safely provide patient care
