A Brief Overview
Manages all aspects of the patient hospital referral process including matching patient needs with Avera@Home services, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the discharge risk tool and weekend processes. Supervisor focuses on the work of the department, quality monitoring, teaching and serving as an ongoing resource.
What you will do
- Oversees daily work flow planning, performs daily patient referral operations, provides direct oversight of the establishment and implementation of referral policies.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, care transitions and hospice.
- Ensures collaborative teamwork daily with Case Managers, Physicians, Social Work and all involved with the Discharge Planning process.
- Leads the implementation of improved work methods and procedures to ensure patients are referred in accordance with policy.
- Gathers and reports referral statistics including key customer referral trends.
- Responsibilities include interviewing, hiring, developing, training, and retaining employees; planning, assigning, and leading work; appraising performance; rewarding and coaching employees; addressing complaints and resolving problems.

Required Education, License/Certification, or Work Experience:
- Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire
Preferred Education, License/Certification, or Work Experience:
- 1-3 years home health/hospice experience
