Essential Job Functions
As an RN Case Manager, you will play a vital role in providing patient-centered care while ensuring clients receive the support they need in their homes and communities.
🔹 Conduct comprehensive assessments and develop individualized care plans.🔹 Provide nursing care, medication administration, and health education.🔹 Monitor and reassess patient needs, adjusting care plans accordingly.🔹 Collaborate with physicians, families, and support staff.🔹 Supervise PCA/home health aides and mentor new RN Case Managers.🔹 Maintain accurate documentation and fulfill certification/recertification requirements.🔹 Participate in on-call duties and quality improvement initiatives.
Requirements
✔ Active Registered Nurse (RN) License✔ Two-Step TB Test (we provide testing if needed)✔ DHS Criminal Background Clearance (we provide screening)✔ Valid Driver’s License & Auto Insurance
