Salary Range: $75,000 – $85,000 annually (DOE)
  • Mileage reimbursement 
  • Full benefits package
What You’ll Do:
  • Manage and coordinate the care of patients in their homes
  • Perform initial assessments, recertifications, develop care plans, and provide skilled nursing care
  • Collaborate with physicians, therapists, other community partners, and caregivers to ensure optimal outcomes
  • Educate patients and families on managing conditions and medications
  • Document accurately and timely in the EMR
  • Participate in case conferences and team meetings
  • SN visits as needed
What We’re Looking For:
  • Current RN license in Colorado (BSN preferred)
  • Minimum 1 year of nursing experience (home health or acute care preferred)
  • Strong assessment, clinical, and communication skills
  • Ability to work independently while being part of a team
