Role Snapshot

• RN Case Manager – Home Health
• $40/hr
• Full-time (36–40 hrs/week)
• Onsite
• Home visits across the DFW region
• One patient at a time. Clear plans. Stable hours. No experience needed—new grads welcome.

What You'll Do

• Deliver in-home skilled nursing care

• Manage care plans with clear protocols

• Monitor vitals, meds, equipment

• Document in Kantime EMR (modern system)

• Coordinate with families and clinical team

• Support safe transitions and continuity of care

Must-Haves

• Active Texas RN license

• Able to work 36–40 hrs/week

• Willing to complete home visits in your county/neighboring counties

• Comfortable with home health environments

• Work authorization in the U.S.

Nice-to-Have

• Kantime EMR or EVV experience

• Home health background

• Bilingual (Spanish)
